Membership Registration

Welcome to the Avink Funeral Home Cremation Society membership registration page. Registration is completely voluntary, but offers a unique means of ensuring that your wishes are taken care of.

As member, your information will be kept on file to ensure your family acknowledges your wishes and experiences a seamless process during a difficult time.

Registration fees are $15 for single and $25 for a married couple.

This is a one time fee and will ensure your information remains on file at the funeral home.

Please download and print the following form and mail your check to the address listed within. Upon receipt of your registration and payment, you will be sent a certificate and wallet card to acknowledge your registration and membership.
